- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree with completion of an accredited urology residency
- Board certification or eligibility in Urology (ABU or equivalent)
- Valid and unrestricted medical license in the assignment state
- Current DEA registration
- Strong clinical background in urological diagnosis and treatment
