As a Social Care Navigator (CHW), you will play a crucial role in supporting our members, particularly those experiencing social, behavioral, or environmental challenges that impact their overall well-being. Our organization partners with insurance companies, government agencies, universities, non-profits, and employers to provide subsidized, medically tailored meals and healthy grocery deliveries, ultimately making a real difference in the lives of our members.
As a Social Care Navigator (CHW), you will leverage your expertise in motivational interviewing and your understanding of community-based resources to guide members toward their health, housing, nutrition, and personal goals. This position is designed for individuals who are passionate about health equity, social care, and empowering people to build confidence as they navigate complex care journeys.
In this contract role, you will work closely with members remotely, providing structured outreach by phone, text, or video. Your responsibilities will include identifying and referring individuals to essential local resources, collaborating with internal teams, and accurately documenting care plans. Social Care Navigator (CHW) - Summary
- Support members facing social, behavioral, or environmental barriers to health and well-being.
- Utilize motivational interviewing to empower members to set and achieve goals.
- Guide individuals to appropriate community resources and services for their needs.
- Collaborate with internal teams to provide seamless, member-centered care.
- Document interactions and care plans using digital platforms.
- Role is contract-based with a 35-hour work week.
Duties & Responsibilities
- Apply motivational interviewing techniques to build trust and support positive change.
- Conduct structured outreach to members via phone, text, or video.
- Assess member needs and provide personalized referrals to resources such as food assistance, housing support, transportation, and behavioral health services.
- Work collaboratively with team members including Dietitians, Member Support, and Clinical Partners for continuity of care.
- Accurately document all member interactions, care plans, and progress in internal systems.
- Consistently follow up with members to monitor progress, address barriers, and provide encouragement.
- Deliver care that is trauma-informed, culturally responsive, and grounded in empathy and respect.

Qualifications & Requirements
- Minimum of 2 years' experience in care navigation, social work, health coaching, behavioral health, or a related support role.
- Community Health Worker (CHW) certification is a plus.
- Proficiency in motivational interviewing or similar approaches that facilitate positive change.
- Familiarity with Medicaid, SNAP, public housing, or behavioral health services preferred.
- Experience supporting low-income individuals or those with chronic health conditions.
- Comfortable working remotely and utilizing digital tools such as CRM or EHR for documentation.
- Willingness to travel occasionally for in-person training as needed.
