Social Care Navigator – Spanish Speaking
Tangelo is seeking a dedicated Social Care Navigator to support individuals facing social, behavioral, or environmental challenges impacting their health and well-being. As a Social Care Navigator, you will play a pivotal role in connecting members with essential services and guiding them on their journey to improved health outcomes. This position is particularly focused on serving Spanish-speaking members and requires fluency in Spanish. The Social Care Navigator role offers a unique opportunity to make a direct impact through Motivational Interviewing, personalized support, and collaboration with a passionate, mission-driven team. This is a contract-based, full-time role designed for those who thrive in a remote-first work environment and are committed to advancing health equity for vulnerable populations.

Social Care Navigator - Summary
- Serve as a trusted resource for members, especially those facing barriers related to social determinants of health.
- Utilize Motivational Interviewing techniques to empower members and support their health, housing, nutrition, and personal goals.
- Fluency in Spanish is required to effectively support our Spanish-speaking member population.
- Collaborate with a multidisciplinary team to ensure continuity and quality of care.
- Remote-first employment with occasional in-person meetings for training and team collaboration.
Duties & Responsibilities
- Build trustful relationships with members by applying Motivational Interviewing strategies, fostering empowerment and self-efficacy.
- Conduct structured outreach through phone, text, or video to assess member needs and provide tailored support.
- Identify community and governmental resources, such as food assistance, housing programs, transportation, and behavioral health services, and refer members appropriately.
- Work collaboratively with internal teams, including Dietitians, Member Support, and Clinical Partners, ensuring seamless handoffs and integrated care plans.
- Accurately document all member interactions, goals, and progress using Tangelo’s internal systems.
- Consistently follow up with members to monitor progress, remove obstacles, and celebrate achievements.
- Deliver trauma-informed, culturally responsive care grounded in empathy and respect for all members.

Qualifications & Requirements
- Minimum of 2 years’ experience in Care Navigation, Social Work, Health Coaching, Behavioral Health, or a related support role.
- Community Health Worker certification is considered an asset.
- Fluency in Spanish (required).
- Demonstrated skill in Motivational Interviewing or similar change-supportive methodologies.
- Familiarity with public health and social service systems such as Medicaid, SNAP, public housing, and behavioral health services.
- Experience supporting low-income individuals or those living with chronic health conditions.
- Comfort with remote work and proficiency in using digital tools, including CRM and EHR systems, for documentation and communication.
- Willingness to participate in occasional in-person training sessions as needed.
