Skilled Nursing Facility Physician (SNF Physician) – Summary- Deliver primary care services to seniors in skilled nursing facilities
- Work collaboratively with an experienced clinical and administrative support team
- Practice in an environment where patient care quality and team-based approaches are prioritized
- Contribute to a mission-driven organization dedicated to improving outcomes for the aging population
- Conduct comprehensive medical assessments for skilled nursing facility residents
- Develop, implement, and monitor individualized care plans in coordination with interdisciplinary teams
- Provide ongoing management of chronic conditions, acute illnesses, and medication regimens
- Collaborate with nursing staff, therapists, case managers, and family members to ensure optimal patient outcomes
- Participate in care conferences and contribute medical expertise to the care team
- Maintain accurate and timely documentation of patient encounters and administrative duties
- Engage in quality improvement activities to support best practices and regulatory compliance
- Serve as a clinical resource and mentor to less experienced providers or staff as needed
- Competitive total compensation package exceeding $375,000
- Balanced schedule with 25 patient-facing hours and 15 administrative hours weekly
- Low daily patient volume, enabling relationship-based and comprehensive care

- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Current Colorado medical license, or eligibility to obtain prior to start date
