Skilled Nursing Facility Physician (SNF Physician) – Summary
- Deliver primary care services to seniors in skilled nursing facilities

Duties & Responsibilities
- Conduct comprehensive medical assessments for skilled nursing facility residents
- Develop, implement, and monitor individualized care plans in coordination with interdisciplinary teams
- Provide ongoing management of chronic conditions, acute illnesses, and medication regimens
- Collaborate with nursing staff, therapists, case managers, and family members to ensure optimal patient outcomes
- Participate in care conferences and contribute medical expertise to the care team
- Maintain accurate and timely documentation of patient encounters and administrative duties
- Engage in quality improvement activities to support best practices and regulatory compliance
- Serve as a clinical resource and mentor to less experienced providers or staff as needed

- Balanced schedule with 25 patient-facing hours and 15 administrative hours weekly

Qualifications & Requirements
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Current Colorado medical license, or eligibility to obtain prior to start date
