As a Registered Nurse Special Needs Coordinator (RN SNC), you will oversee and manage the installation Exceptional Family Member Program – Medical (EFMP-M) in accordance with Department of Defense (DoD) and Air Force (AF) policies. Your responsibilities will include clinical assessment, identification, and support for families with special needs, as well as collaboration with key service providers for effective outreach and care coordination. The RN SNC plays a critical role in ensuring that all active-duty sponsors with family members who have special needs are properly identified and enrolled in AF medical special needs data management systems. Additionally, you will provide clinical training to installation personnel and support implementation of the EFMP-M across the base.

Registered Nurse Special Needs Coordinator (RN SNC) - Summary
- Serve as the primary coordinator for the installation’s EFMP-M program.
- Utilize advanced nursing skills to identify, enroll, and support families with special medical and educational needs.
- Collaborate closely with military and civilian agencies to ensure comprehensive care delivery.
- Provide vital clinical training and oversight to installation personnel and healthcare teams.
Duties & Responsibilities
- Manage and oversee the local execution of the EFMP-M program according to DoD and AF policies.
- Perform clinical assessments to identify sponsors and family members with special needs in a timely manner.
- Collaborate with service providers, youth organizations, and spouse groups for outreach and support.
- Ensure accurate and timely data entry and management within AF-provided systems for EFMP-M.
- Provide clinical training, consultation, and information to installation personnel and medical staff.
- Oversee the Family Member Relocation Clearance (FMRC) process and ensure compliance with all requirements.
- Coordinate enrollment and travel screening for military families relocating, especially to overseas bases.
- Maintain up-to-date, confidential Special Needs files and ensure annual sponsor contact for needs assessment.
- Participate in quarterly case reviews and multidisciplinary meetings to address complex or unmet clinical needs.
- Ensure compliance with all relevant local, DoD, and AF policies, including documentation and HIPAA regulations.
- Contribute to the orientation and training of other Special Needs Program staff.
- Engage in continuous quality improvement, productivity, and professional standards of practice.
- Travel as required to paired communities and medical facilities using government transport as needed.

Qualifications & Requirements
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution recognized by the US Department of Education.
- Active, valid, and unrestricted Registered Nurse (RN) license in any US state/jurisdiction.
- Basic Life Support (BLS) certification is required.
- Current with all continuing education requirements for nursing licensure.
- Minimum of three years (for ADN) or two years (for BSN) of full-time nursing and care coordination experience.
- U.S. Citizenship is required.
