The Registered Nurse – Special Needs Coordinator (RN, SNC) is a vital healthcare professional responsible for overseeing and managing the Exceptional Family Member Program-Medical (EFMP-M) in accordance with Department of Defense (DoD) and Air Force (AF) policies. The RN, SNC acts as a liaison between military families, medical staff, installation personnel, and various support agencies to facilitate the provision of comprehensive care and services for those enrolled in the EFMP-M program.
This role is deeply collaborative and involves multi-disciplinary teamwork to ensure all active duty sponsors with family members requiring special medical or educational attention are appropriately identified, enrolled, and supported through ongoing clinical oversight and outreach. The Registered Nurse – Special Needs Coordinator (RN, SNC) also provides essential training and consultation to installation and medical staff to maintain high standards in program implementation and family support.
Working in this capacity, the RN, SNC engages in ongoing data management, maintains accurate clinical documentation, and upholds all standards of professional nursing practice within the military healthcare system. The position also involves coordination with the Integrated Delivery System (IDS), participation in case reviews, and collaboration with both base and civilian agencies to ensure families have access to necessary resources and referrals. Registered Nurse – Special Needs Coordinator (RN, SNC) Summary
- Oversees the implementation and management of the Exceptional Family Member Program-Medical (EFMP-M) at the installation level
- Serves as a clinical resource for identifying, enrolling, and supporting military families with special needs
- Collaborates with internal and external service providers to ensure effective outreach and support
- Provides clinical training to staff and maintains data integrity within AF medical special needs systems
Duties & Responsibilities
- Manage the installation EFMP-M program in compliance with DoD and AF policies
- Utilize clinical assessment skills for timely identification of special needs among family members
- Coordinate with installation youth services, childcare facilities, and spouse groups for outreach
- Ensure all sponsors with special needs family members are identified and enrolled in appropriate systems
- Oversee base-level data entry and maintain clinical documentation in electronic medical records (AHLTA, EMR)
- Provide clinical training and consultation for installation personnel and medical staff
- Support the Family Member Relocation Clearance (FMRC) process and ensure compliance with all related requirements
- Coordinate travel screening for family members assigned to overseas bases
- Maintain annual contact with Q-coded sponsors to identify unmet needs and update records
- Collaborate with the Integrated Delivery System (IDS) and other agencies for information and referrals
- Refer EFMP families to additional support resources as needed
- Participate in EFMP quarterly case reviews and various facility meetings, committees, or work groups

Qualifications & Requirements
- Active, valid, unrestricted Registered Nurse (RN) license in any US state or territory, in good standing
- Graduate of an accredited Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) program
- Completion of all continuing education required for licensure
- Minimum of three years' full-time nursing and care coordination experience for ADN graduates; two years for BSN graduates
- Experience in clinical assessment, care coordination, and multidisciplinary teamwork
- Ability to travel as needed, sometimes using Government Owned Vehicles (GOV)
- Compliance with all relevant federal, DoD, and facility-specific policies and procedures
