As a Registered Nurse Case Manager (RN, TBI, CCM), you will play an essential role in managing the care of patients with complex medical needs, particularly those affected by traumatic brain injury (TBI). You will collaborate with multidisciplinary teams to coordinate assessments, referrals, treatment plans, and transitions of care, ensuring all activities adhere to established standards and guidelines from organizations such as BUMED and DHA. Your contributions will directly impact patient outcomes, quality of care, and efficient utilization of healthcare resources.

Working in this capacity, you will leverage your clinical expertise to act as both a patient advocate and a resource for healthcare teams. Your responsibilities will include direct patient interaction, comprehensive case management, and data tracking related to TBI care. You will facilitate communication among healthcare professionals, patients, and families, ensuring that each patient receives individualized, patient-centered, and cost-effective care.
