As a Registered Nurse Case Manager (RN, TBI, CCM), you will play an essential role in managing the care of patients with complex medical needs, particularly those affected by traumatic brain injury (TBI). You will collaborate with multidisciplinary teams to coordinate assessments, referrals, treatment plans, and transitions of care, ensuring all activities adhere to established standards and guidelines from organizations such as BUMED and DHA. Your contributions will directly impact patient outcomes, quality of care, and efficient utilization of healthcare resources.
Working in this capacity, you will leverage your clinical expertise to act as both a patient advocate and a resource for healthcare teams. Your responsibilities will include direct patient interaction, comprehensive case management, and data tracking related to TBI care. Registered Nurse Case Manager (RN, TBI, CCM) - Summary
- Deliver comprehensive case management services to patients with traumatic brain injury and complex medical needs.
- Coordinate multidisciplinary care, including assessments, referrals, and transitions across healthcare settings.
- Ensure compliance with established standards, BUMED/DHA guidelines, and quality documentation requirements.
- Serve as a clinical resource and advocate for patients and families.
- Track and analyze outcomes and data related to TBI management.
Duties & Responsibilities
- Oversee all phases of case management activities for assigned caseloads, ensuring adherence to case management standards of care.
- Provide independent nursing assessments, data collection, and analysis for patients with TBI and other complex needs.
- Develop individualized, multidisciplinary care plans using critical pathways and clinical guidelines.
- Coordinate referrals, ancillary services, and transitions between care settings, including region-to-region transfers.
- Ensure timely and accurate documentation of patient care, consults, and treatment outcomes.
- Facilitate effective communication among care teams, patients, families, and community resources.
- Support cost-effective and quality-conscious use of healthcare resources.
- Participate in team meetings, staff education, and professional development activities.
- Promote continuous quality improvement and safety by evaluating patient care systems and incorporating evaluative data.
- Serve as a liaison to internal staff, community agencies, and referring organizations.
- Support patient and family education, addressing learning needs and preparing caregivers as necessary.
- Assist with the development and dissemination of information for staff and patients.
- Promote effective communication and collaboration within multidisciplinary teams.

Qualifications & Requirements
- Current unrestricted license as a Registered Nurse (RN) in the United States or its territories.
- Bachelor of Science in Nursing (BSN) from an accredited institution (NLNAC or CCNE).
- Basic Life Support (BLS) certification required.
- One of the following certifications (or the ability to obtain within 6 months of hire): Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Commission on Rehabilitation Counselor Certification (CRC), Advanced Certification in Continuity of Care (ACCC), Certified Occupational Health Nurse (COHN or COHN-S), or Certified Rehabilitation Registered Nurse (CCRN).
- At least 2 years of recent inpatient medical-surgical or acute rehabilitation experience within the last 3 years.
- U.S. Citizenship required.
