As a Registered Nurse Case Manager (RN) – Special Needs Coordinator, you will play a crucial role in supporting the Exceptional Family Member Program Medical (EFMP-M), ensuring that families with special medical and educational needs receive the highest standard of care and support. You will work collaboratively within a multidisciplinary framework, interacting with key service providers, medical staff, and military personnel to manage and coordinate care delivery. The position places you at the heart of program implementation and oversight, leveraging your clinical expertise to identify, assess, and meet the needs of special populations while maintaining compliance with Department of Defense (DoD) and Air Force (AF) policies.
Your daily responsibilities will be varied and impactful, requiring the application of your Bachelor of Science in Nursing (BSN) and Basic Life Support (BLS) certification, as well as your experience in care coordination, discharge planning, or case management. In this role, you will establish and maintain effective procedures to identify and support families with special needs, engage with both internal and external partners, and provide training and consultation to ensure consistent program delivery. Registered Nurse Case Manager (RN) – Special Needs Coordinator Summary
- Serve as the lead coordinator for the Exceptional Family Member Program Medical (EFMP-M).
- Collaborate with multidisciplinary teams to identify and support military families with special medical and educational needs.
- Ensure program compliance with DoD and Air Force policies, consistently applying best practices in case management.
- Provide oversight and consultation to ensure families receive appropriate resources and services.
Duties & Responsibilities
- Oversee and manage the EFMP-M in accordance with Department of Defense and Air Force guidance.
- Establish procedures to identify family members with special needs promptly and efficiently.
- Utilize clinical assessment skills to maintain accurate identification and record-keeping for sponsors and families.
- Lead collaborative initiatives with installation service providers, youth and childcare facilities, and support groups to ensure effective outreach and identification of special needs.
- Ensure accurate data management and oversight of family member information in Air Force medical special needs data systems.
- Provide training and consultation to medical staff and installation personnel for base-wide EFMP-M implementation.
- Participate in and support the Family Member Relocation Clearance (FMRC) process, coordinating enrollment and travel screening for military families.
- Maintain special needs files for all sponsors and ensure annual contact to assess unmet needs and update information as required.
- Collaborate with integrated delivery systems and interagency forums to provide family medical information and facilitate community referrals.
- Support the integration of EFMP-M with related family support and assignment programs at the installation.
- Participate in weekly care management reviews to assess new and complex cases, ensuring that families receive appropriate resources and support.

Qualifications & Requirements
- Bachelor of Science in Nursing (BSN) required.
- Current Basic Life Support (BLS) certification.
- At least one year of full-time experience in care coordination, discharge planning, or case management in a medical setting within the last four years.
- Strong clinical assessment and organizational skills.
- Experience working with multidisciplinary teams and diverse populations.
