Primary Care Physician (PCP) – Internal Medicine / Family Medicine Summary
- Provide comprehensive outpatient primary care to the senior population.
- Practice in a value-based care model focused on patient outcomes and satisfaction.
- Work as part of an interdisciplinary team including medical assistants, referral specialists, nurses, case managers, social workers, and outreach specialists.
- Participate in innovative community health programs and patient engagement activities.
Duties & Responsibilities
- Deliver evidence-based primary care to a dedicated panel of senior patients, averaging 10–13 patients per day.
- Conduct patient assessments, develop care plans, and provide routine follow-up visits to ensure optimal health outcomes.
- Utilize a value-based care approach to improve patient satisfaction and health results.
- Collaborate with a multidisciplinary care team to coordinate comprehensive care, including labs, EKG, and echo services available onsite.
- Engage in community initiatives such as cooking classes, yoga, exercise programs, and social events aimed at promoting senior wellness and connection.
- Participate in initial and ongoing training programs to stay current with best practices in primary care.
- Chart patient interactions during office hours, promoting work-life balance.
- Take part in a shared call rotation (1:8 weeks) with a low volume of calls per week.
- Ensure compliance with all organizational policies and applicable regulations.
- Support care coordination by arranging transportation for patients as needed, leveraging partnerships with ride-sharing services.

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine (IM) or Family Medicine (FM).
- Strong computer proficiency and comfort with electronic medical records.
- No current legal actions, impairment, or violations related to Medicare, fraud, anti-kickback statutes, or professional conduct.
- Open to candidates completing residency in the near future.
