Primary Care Physician (PCP) - Summary
- Deliver high-quality, value-based care to Medicare patients in a neighborhood primary care center.
- Collaborate with experienced care teams to optimize patient outcomes and experiences.

Duties & Responsibilities
- Conduct regular and comprehensive patient visits (average 13-16 patients/day) with individualized care plans.
- Utilize on-site labs, phlebotomist services, and digital health platforms such as Canopy and Greenway EMR systems.
- Work alongside medical assistants, scribes, social workers, and other care team members to provide coordinated care.
- Participate in quality improvement initiatives and value-based care programs focused on preventive health and reduced hospitalizations.
- Engage patients in community activities and wellness programs hosted in the clinic’s community room.
- Access specialist consultations via RubiconMD as needed for comprehensive care provision.
- Support patient transportation needs within a five-mile radius for improved access to care.

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing (non-probationary).
- Fellowship training in Geriatrics preferred, but not required.
- No residents; only fully licensed physicians will be considered.
