Primary Care Physician (PCP) – Full-time in Houston, TX

Primary Care Physician (PCP) - Summary
- Full-time Primary Care Physician (PCP) role in a value-based care setting
- Focus on adult and geriatric populations, mainly Medicare Advantage patients
- Integrated care team structure including nurse practitioners, physician assistants, social workers, and support staff
- Opportunity to make a meaningful impact on senior health outcomes
Duties & Responsibilities
- Provide comprehensive primary care services to adults 65+, with a focus on chronic disease management and preventive care
- See 12–15 patients per day, allowing for in-depth consultations (20–40 minutes per visit)
- Participate in coordinated care efforts with nurse practitioners, physician assistants, social workers, pharmacy, lab, and referral coordinators
- Utilize electronic medical records (EMR) efficiently, with training and support provided (currently transitioning to Athena)
- Contribute to an environment dedicated to value-based outcomes, quality care, and patient satisfaction
- Engage in continuous professional development, including a comprehensive 3-week physician onboarding program

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience with geriatric populations is highly preferred
- Prior exposure to Value-Based Care (VBC) models or HEDIS metrics is a plus, but not required
- Commitment to delivering patient-focused, high-quality care
- Strong interpersonal, communication, and teamwork skills
