Primary Care Physician (PCP) – Outpatient, Full-Time

This full-time Primary Care Physician (PCP) role is focused on delivering comprehensive, proactive care for an adult population, primarily Medicare Advantage patients. You will benefit from an integrated care team and advanced practice support that allows you to spend more time with each patient—typically 20 to 40 minutes per visit—with a manageable daily census of 12 to 15 patients. Primary Care Physician (PCP) - Summary- Outpatient, full-time Primary Care Physician (PCP) position focused on adults 65+ and Medicare Advantage patients
- Value-based care model emphasizing preventive medicine and chronic disease management
- Collaborative integrated care team environment with clinical support
- Emphasis on quality patient interactions and relationship-building
- Provide comprehensive primary care services for adults aged 65 and older in an outpatient setting
- Manage a daily census of 12–15 patients, allocating 20–40 minutes per patient visit
- Focus on preventive care, chronic disease management, and Medicare Advantage population health outcomes
- Collaborate closely with an integrated team including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, lab, pharmacy, and referral coordinators
- Document patient encounters accurately in the EMR system (Athena, with ongoing optimization and training provided)
- Participate in a comprehensive 3-week onboarding and training program, including value-based care best practices and workflow optimization
- Engage in continuous professional development and quality improvement initiatives
- Ensure compliance with all regulatory and organizational standards
- Highly competitive base salary, commensurate with experience
- Total first-year compensation package exceeding $300,000, including performance and sign-on bonuses
