This full-time Primary Care Physician (PCP) role is focused on delivering comprehensive, proactive care for an adult population, primarily Medicare Advantage patients. Primary Care Physician (PCP) - Summary
- Outpatient, full-time Primary Care Physician (PCP) position focused on adults 65+ and Medicare Advantage patients
- Value-based care model emphasizing preventive medicine and chronic disease management

Duties & Responsibilities
- Provide comprehensive primary care services for adults aged 65 and older in an outpatient setting
- Manage a daily census of 12–15 patients, allocating 20–40 minutes per patient visit
- Focus on preventive care, chronic disease management, and Medicare Advantage population health outcomes
- Collaborate closely with an integrated team including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, lab, pharmacy, and referral coordinators
- Document patient encounters accurately in the EMR system (Athena, with ongoing optimization and training provided)
- Participate in a comprehensive 3-week onboarding and training program, including value-based care best practices and workflow optimization
- Engage in continuous professional development and quality improvement initiatives
- Ensure compliance with all regulatory and organizational standards
Salary & Benefits
- Highly competitive base salary, commensurate with experience
- Total first-year compensation package exceeding $300,000, including performance and sign-on bonuses

Qualifications & Requirements
- MD or DO degree, board certified or board eligible in Internal Medicine or Family Medicine
- Experience caring for geriatric patients strongly preferred
- Value-based care experience is a plus; comprehensive training provided for motivated candidates
- HEDIS experience desirable
- Commitment to patient-centered, high-quality care in a collaborative team environment
- Strong interpersonal, communication, and organizational skills
- Eligible for unrestricted medical licensure
