Primary Care Physician (PCP) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 400 patients, primarily seniors aged 65+
- Conduct patient visits, manage chronic conditions, and address acute health concerns
- Utilize a value-based care approach to optimize health outcomes and patient satisfaction
- Work collaboratively with a multidisciplinary care team including nurses, case managers, social workers, and outreach specialists
- Perform on-site diagnostic procedures such as EKGs and laboratory testing
- Complete patient charting during office hours to support work-life balance
- Participate in initial and ongoing training programs
- Contribute to community wellness through organizing and participating in classes and social activities for seniors
- Maintain compliance with all regulatory and ethical standards
- Participate in a call rotation, averaging 3-5 calls during assigned weeks (call rotation is 1:8 weeks)

Qualifications & Requirements
- Board-certified or board-eligible in Internal Medicine (IM) or Family Medicine (FM)
- Proficiency with electronic health records and computer systems
- No current legal or professional disciplinary actions
- Eligible for Medicare participation without restrictions
- Open to recent residency graduates
