We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our patient-centered outpatient practice focused on delivering exceptional value-based care to seniors. This full-time position offers an opportunity to provide comprehensive primary healthcare services, leveraging a concierge-style approach to ensure high-quality outcomes for patients aged 65 and older. Our mission is to bridge the gap in access to premium primary care for those who may not otherwise afford such services, while also supporting physician work-life balance and professional growth.
As a Primary Care Physician (PCP), you will be part of a collaborative care team, working alongside Medical Assistants, Referral Specialists, Nurses, Case Managers, Social Workers, Outreach Specialists, and community health coordinators. With a manageable patient panel and a supportive work environment, you will have the ability to deliver personalized care, build meaningful patient relationships, and enjoy a robust support network within the practice. Onsite resources such as EKGs, laboratory services, and community wellness initiatives further enhance your ability to provide holistic care.

Primary Care Physician (PCP) - Summary
- Role: Provide outpatient primary care to seniors in a value-based care model
- Employment Type: Full-time
- Patient Panel: Max of 400 patients per physician

Duties & Responsibilities
- Deliver comprehensive primary healthcare to patients aged 65 and older in an outpatient setting
- Practice at the top of your medical license within a value-based care framework
- Complete clinical charting during office hours to support work-life balance
- Participate in patient-centered initiatives, including wellness programs, health education, and community events
- Coordinate with a multidisciplinary care team including Medical Assistants, Referral Specialists, Nurses, Case Managers, Social Workers, and Outreach Specialists
- Perform diagnostic procedures such as EKGs and oversee laboratory work onsite
- Maintain a low volume, high-quality patient schedule to ensure thorough and attentive care
- Engage in ongoing training and professional development opportunities provided by the organization
- Participate in a rotating call schedule (1:8 weeks), with a manageable number of after-hours calls
- Support seamless patient transportation and access to specialist care through established contracts with local providers

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine (IM) or Family Medicine (FM)
- Strong computer skills and comfort with electronic medical record systems
- Clear professional record with no current cases, impairment, sexual misconduct, anti-kickback, or fraud violations
- No restrictions with Medicare
