Primary Care Physician (PCP) - Summary
- Provide comprehensive outpatient primary care services to a Medicare Advantage patient panel.
- Practice in a value-based care model emphasizing prevention, chronic care, and patient-centered outcomes.
- Work within a modern facility equipped to support holistic patient wellness.
- Collaborate with a multidisciplinary care team for optimal patient results.
Duties & Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Manage a patient volume of 14-16 individuals per day, allowing for longer and higher-quality appointments.
- Coordinate preventive care, chronic disease management, and wellness activities for patients.
- Work closely with care team members including medical assistants, behavioral health specialists, social workers, and registered nurses.
- Utilize electronic health records (EHR) to document patient encounters accurately and efficiently.
- Participate in ongoing quality improvement initiatives and meet established performance metrics.
- Engage in collaborative case reviews and team meetings to continuously enhance patient care.
- Educate patients and their families on health maintenance, prevention, and treatment options.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
