Primary Care Physician (PCP) – Summary- Full-time Primary Care Physician (PCP) position focused on adults aged 65+
- Value-based care model emphasizing preventive and chronic disease management

- Provide high-quality, patient-centered primary care for adults 65+, primarily Medicare Advantage patients
- Conduct thorough preventive and chronic disease management visits, with 20–40 minutes allocated per patient
- Manage a daily census of 12–15 patients for optimal quality of care
- Collaborate with an integrated care team including NPs/PAs, medical assistants, social workers, and pharmacy/lab support
- Coordinate referrals, transitions of care, and follow-up to ensure continuity and quality outcomes
- Participate in comprehensive onboarding, including a 3-week physician learning journey focused on value-based care, documentation, and EMR optimization
- Engage in ongoing training and professional development initiatives
- Champion a mission-driven, impact-focused approach to improving senior health outcomes
- Base salary range: $235,000–$270,000, commensurate with experience
