Primary Care Physician (PCP) - Summary- Deliver comprehensive outpatient care to a senior patient population with multiple chronic conditions
- Work within a physician-led, patient-first culture focused on quality and value-based outcomes
- Benefit from a manageable patient panel, allowing for enhanced patient relationships and continuity of care
- Collaborate with a dedicated support team, including medical assistants, schedulers, and referral specialists
- See 18-22 patients per day, maintaining a small patient panel of up to 450 individuals
- Conduct comprehensive assessments and develop individualized care plans for patients, focusing on seniors with 3-5 chronic conditions
- Utilize value-based care principles to drive improved health outcomes and patient satisfaction
- Work Monday through Friday, with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm)
- Participate in administrative time built into your schedule to support high-quality care delivery
- Work exclusively in an outpatient setting with no nights or in-hospital call responsibilities
- Collaborate with in-house specialists in areas such as cardiology and podiatry
- Engage with holistic health services offered in the clinic, including acupuncture, tai chi, yoga, and nutrition counseling
- Competitive base salary starting at $260,000, plus up to 10% performance-based bonus
- Potential signing bonus up to $50,000
- Comprehensive benefits package, including medical, dental, vision, and supplemental plans for providers and families
- 401(k) retirement plan with a 5.5% employer match
- Paid time off (PTO) totaling 33 days annually
- Additional $3,500 and one week for continuing medical education (CME)
- Work-life balance supported by no night shifts, in-hospital call, or weekend requirements
- Residency-trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
