The Primary Care Physician (PCP) will manage a panel of senior patients, most with multiple chronic conditions, ensuring comprehensive and compassionate care. The practice operates on a standard weekday schedule, with no nights required, and provides ample administrative time within the workday. Our integrated care model ensures that you have access to in-house consulting specialists and a holistic health services team, enabling you to meet the full spectrum of your patients’ needs. Dedicated support staff—including medical assistants, schedulers, and referral specialists—help streamline patient management and ensure seamless care coordination.

Primary Care Physician (PCP) - Summary- Full-time role providing primary care services to a senior patient population
- Physician-led, mission-driven organization focused on value-based care

- Robust administrative and clinical support teams
- Deliver comprehensive primary care to a panel of approximately 450 senior patients
- Manage and treat patients with multiple chronic conditions (average 3-5 per patient)
- See 18-22 patients per day in an outpatient setting
- Work Monday through Friday; last patient scheduled at 4:15 PM
- Participate in value-based care initiatives and quality improvement programs
- Collaborate with in-house consulting specialists such as cardiologists and podiatrists
- Utilize holistic health services including acupuncture, yoga, and nutrition counseling
- Coordinate with dedicated support staff for efficient patient care
- Maintain accurate and thorough documentation in the electronic health record

- Medical degree (MD or DO) from an accredited institution
- Residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Current and unrestricted medical license

- Experience or strong interest in caring for senior populations
- Demonstrated ability to work effectively in a team-based care model
