The Primary Care Physician (PCP) plays a pivotal role in delivering high-quality, value-based healthcare to Medicare patient populations through an innovative network of neighborhood primary care centers. The focus of the PCP is to ensure equitable, effective, and patient-centered care, working alongside a dedicated care team to drive improved health outcomes and enhance the patient experience.
Within our progressive value-based care model, the Primary Care Physician is empowered to prioritize preventive medicine, reduce hospital admissions, and provide comprehensive care without the pressure of volume-based metrics. This approach supports a more meaningful provider-patient relationship and rewards clinical outcomes, making it an ideal environment for physicians who are passionate about impactful care delivery.
Supported by a multidisciplinary care team—including scribes, medical assistants, and social workers—the PCP is able to focus on patient needs, supported by robust onsite resources such as laboratory facilities and specialist teleconsultation via RubiconMD. The daily patient volume is intentionally managed to allow for thorough consultations, with regular visits scheduled for 20 minutes and new or comprehensive diagnostic appointments allocated 40 minutes.
The Primary Care Physician works collaboratively within a clinic staffed by three to five providers, including both Nurse Practitioners (NPs) and fellow Primary Care Physicians. This collaborative environment fosters continual learning and professional support, with the added benefit of community engagement activities such as exercise classes, wellness events, and patient social gatherings organized within the clinic’s community room.
The clinic leverages electronic medical record systems such as Canopy and Greenway, streamlining documentation and coordination of care. Additionally, transportation assistance is provided for patients within a five-mile radius, ensuring accessibility and continuity of care for the local population.

- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree required.
- Board certified or board eligible in Internal Medicine or Family Medicine.

- Fellowship training in Geriatrics preferred but not required.
