Primary Care Physician (PCP) - Summary
- Deliver high-quality, value-based primary care to Medicare patient populations
- Work collaboratively within a team-oriented, patient-centered environment

- Focus on preventive medicine and long-term patient health
Duties & Responsibilities
- Provide comprehensive primary care services to patients, including preventive care, chronic disease management, and acute medical services
- Conduct patient visits with an emphasis on quality of care, not volume, seeing an average of 13-16 patients per day
- Perform regular appointments (20 minutes) and full diagnostic/new patient visits (40 minutes)
- Collaborate with a care team including a scribe, medical assistant, and social worker for each patient
- Utilize on-site labs and phlebotomy services for efficient diagnosis and treatment
- Access specialist consultations via RubiconMD as needed
- Document all patient encounters in Canopy and Greenway EMR systems
- Participate in value-based health initiatives, focusing on outcomes and patient satisfaction
- Engage in community health activities and promote patient participation in wellness events, exercise programs, and educational sessions
- Ensure a collaborative and supportive clinic environment with 3-5 providers per location

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license in good standing
- Bilingual proficiency in Spanish is required
- Fellowship training in Geriatrics is preferred but not required
- Must not be a current resident
