Primary Care Physician (PCP) - Summary- Full-time position focused on primary care for adults aged 65+
- Emphasis on value-based care, preventive medicine, and chronic disease management

- Deliver preventive and comprehensive primary care to a patient panel of adults 65+ (primarily Medicare Advantage)
- Conduct detailed patient assessments, develop individualized care plans, and manage chronic conditions
- Average daily census of 12-15 patients, with 20-40 minutes scheduled per visit for patient-centered care
- Collaborate with NPs, PAs, medical assistants, social workers, and pharmacy/lab staff within an integrated care model
- Provide leadership within the care team, supporting best practices and quality initiatives
- Participate in performance improvement initiatives and adhere to documentation standards
- Engage in professional training, including a comprehensive 3-week onboarding journey covering value-based care, EMR use, and integrated workflows
