As a Primary Care Physician (PCP), you will collaborate within an integrated care team focused on chronic disease management, preventive medicine, and enhancing patient outcomes. You will be supported by nurse practitioners, physician assistants, medical assistants, social workers, and in-house lab and pharmacy professionals to ensure your patients receive holistic, coordinated care. Primary Care Physician (PCP) - Summary- Full-time physician role within a value-based care organization
- Focus on preventive and chronic care management for adults 65+
- Patient-centered, relationship-focused medical practice

- Provide comprehensive primary care services for an adult patient panel, primarily Medicare Advantage beneficiaries
- Conduct preventive screenings, manage chronic conditions, and deliver acute care as needed
- Collaborate with NPs, PAs, social workers, medical assistants, and other care team members to coordinate patient care
- Participate in value-based care initiatives, including quality improvement and population health programs
- Utilize electronic health records and data analytics to drive best practices and optimize patient outcomes
- Educate patients and families on disease prevention, wellness, and treatment plans
- Maintain compliance with organizational protocols and regulatory standards
