Primary Care Physician (PCP) - Full-Time

As a Primary Care Physician (PCP), you will work with an integrated care team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, and laboratory and pharmacy professionals. This collaborative approach enables you to provide holistic care for a diverse patient population, primarily focused on Medicare Advantage patients with chronic conditions and preventive health needs. You will have the opportunity to spend more time with each patient, typically seeing 12 to 15 patients per day, with extended visit times to ensure thorough care and attention.
The organization values continuous learning and offers a comprehensive three-week onboarding program for new physicians. This includes in-depth training on value-based care, best practices in documentation, electronic medical record (EMR) optimization, and integrated care workflows. You will be supported throughout your professional journey, with ongoing access to resources that will enhance your clinical skills and operational knowledge.
We are looking for candidates who are board certified or board eligible in Internal Medicine or Family Medicine. Experience in geriatrics or knowledge of HEDIS measures is preferred, but not required, as extensive support and training are provided. Primary Care Physician (PCP) - Summary- Full-time clinical position focused on preventive and chronic care management for adults 65+

- Manage a patient census of 12-15 patients per day with 20-40 minute visit lengths
