Primary Care Physician (PCP) - Summary- Full-time Primary Care Physician (PCP) position in Fort Lauderdale, FL
- Physician-led, value-based care model
- Focus on serving underserved and vulnerable senior populations with complex chronic conditions
- 100% outpatient care setting
- Provide comprehensive primary care to seniors with multiple chronic conditions in a full-time, outpatient setting
- Manage a patient panel of 18–20 patients per day, focusing on quality and outcomes
- Deliver value-based care that emphasizes patient relationships and long-term health improvement
- Collaborate with a dedicated support team, including a Medical Assistant, scheduler, and referral specialist
- Develop and implement individualized care plans for patients with 3–5+ chronic conditions
- Coordinate care with specialists and other providers as needed
- Participate in continuous quality improvement initiatives
- Utilize electronic medical records (EMR) to document patient encounters and track outcomes
- Competitive base salary starting at $250,000+ per year
- Incentive bonus up to 10% of base salary
- Signing bonus up to $50,000
- Partnership track available for eligible physicians
- Monday–Friday schedule, no weekends required
- Strong clinical and administrative support
- Completed residency in Internal Medicine or Family Medicine
- Board Certified or Board Eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Bilingual in English and Spanish required
- Demonstrated commitment to serving underserved senior populations
- Strong teamwork and communication skills
- Experienced or new graduate Primary Care Physician (PCP) passionate about value-based care
- Comfortable managing complex chronic conditions in a senior population
- Fluent in English and Spanish
