Primary Care Physician (PCP) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to a panel of adult patients, focusing on preventive medicine and chronic disease management
- Manage a patient panel of up to 450 seniors, averaging 15-18 patients per day
- Develop individualized care plans for patients with multiple chronic conditions
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) and utilize holistic health services as appropriate
- Work closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Incorporate value-based care (VBC) principles into daily practice to achieve optimal patient outcomes
- Maintain accurate, up-to-date medical records and documentation
- Participate in regular team meetings to discuss patient care strategies and continuous improvement initiatives
- Educate patients and families on health maintenance, disease prevention, and wellness initiatives
- Utilize administrative time included in the schedule for care coordination and follow-up
Salary & Benefits
- Competitive base salary with earning potential exceeding $300K, including bonuses up to 10% of base
- Signing bonus and clear partnership track (no buy-in required; majority become partners after one year)
- 33 days paid time off (PTO) annually

- Up to $3,500 and one week for continuing medical education (CME)
Qualifications & Requirements
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in IM, FM, or Geriatric Medicine
- Experience in value-based care (VBC) preferred, but not required—training available
- Commitment to quality, patient-centered care for seniors with complex health needs
- Strong communication, interpersonal, and organizational skills
