Primary Care Physician (PCP) - Summary
- Deliver comprehensive primary care to adult and senior patients in an outpatient setting
- Work as part of a multidisciplinary care team dedicated to the physical and emotional well-being of each patient
- Practice in a value-based care environment that emphasizes quality, preventive care, and patient satisfaction

Duties & Responsibilities
- Provide primary care services, including diagnosis, treatment, and management of acute and chronic health conditions for patients aged 65 and older
- Coordinate with a dedicated care team, including nurses, case managers, social workers, and outreach specialists to ensure holistic patient care
- Complete documentation and charting during office hours, leveraging electronic medical records
- Engage in patient education and preventive care initiatives tailored to seniors
- Participate in the center’s community programs, such as cooking classes, yoga sessions, and social events aimed at enhancing patient well-being
- Manage a patient panel with a maximum of 400 patients, averaging 10-13 patients per day
- Provide on-call coverage on a rotational basis (1:8 weeks, with minimal after-hours calls)
- Collaborate with care coordinators to arrange transportation and specialist appointments for patients as needed
- Maintain high standards of patient safety, privacy, and satisfaction

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine (IM) or Family Medicine (FM)
- Strong computer skills and experience with electronic medical records systems
- No current legal or disciplinary issues, including no cases related to impairment, sexual misconduct, anti-kickback violations, or Medicare fraud
- Open to hiring current residents or recent graduates (including 2026 residents)
- Demonstrated commitment to providing compassionate, patient-centered care
