Primary Care Physician (PCP) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to adult patients in an outpatient setting
- Conduct patient assessments, manage diagnoses, develop treatment plans, and perform follow-ups
- Collaborate with multidisciplinary clinical teams to improve patient outcomes
- Maintain accurate and thorough documentation using electronic health records (eClinicalWorks)
- Foster strong patient-provider relationships and promote preventative care initiatives
- Participate in practice improvement activities and quality assurance
- Deliver culturally competent care, with Creole language proficiency a requirement

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certification or eligibility in Family Medicine or Internal Medicine
- Active and unrestricted medical license
- Experience with outpatient primary care preferred
- Proficiency in EMR systems (eClinicalWorks experience advantageous)
- Fluency in Creole required for effective patient communication
- ACN physicians are encouraged to apply
