Primary Care Physician (PCP) - Summary- Full-time Primary Care Physician (PCP) position in Philadelphia, PA
- Deliver value-based primary care to local Medicare populations
- Work in innovative, well-supported neighborhood primary care centers
- Focus on outcomes and patient experience, not patient volume
- Provide comprehensive primary care services to Medicare patient populations
- Conduct patient visits (13-16 patients per day) with scheduled 20-minute regular visits and 40-minute diagnostic/new patient visits
- Work Monday through Friday, 8am–5pm
- Collaborate with a care team including a scribe, medical assistant, and social worker
- Utilize on-site labs and phlebotomy services for efficient diagnostics
- Access specialist consultations via RubiconMD
- Document care using Canopy and Greenway EMR systems
- Engage with patients through community room activities, such as exercise, yoga, and wellness events
- Promote preventive medicine and drive improved patient outcomes under a value-based care model
- Participate in regular case reviews and team meetings

- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) graduate
- Board certification (or board eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- No residents at this time
- Fellowship training in Geriatrics preferred, but not required
- Experienced Primary Care Physician (PCP) dedicated to value-based healthcare
- Committed to patient-centered, preventive medicine
- Strong communicator, collaborative, and adaptable to team-based care
- Comfortable using EMR systems and working with diverse patient populations
- Passionate about improving health outcomes and patient satisfaction
- Average care team per clinic includes 3-5 providers (mix of NPs and Primary Care Physicians)
- Transportation support for patients within five miles of clinic
- Access to a variety of patient engagement programs and wellness activities
