Primary Care Physician (PCP) - Summary
- Full-time Primary Care Physician (PCP) position focused on value-based care for seniors.
- Located in Cleveland, Ohio, serving a vibrant and diverse senior community.

Duties & Responsibilities
- Deliver comprehensive primary care to a senior patient panel of up to 450 individuals, managing patients with 3-5 chronic diseases.
- Conduct 18-22 patient visits per day in a 100% outpatient setting.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to provide seamless patient experiences.
- Maintain a work schedule of Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- Utilize administrative time included in your schedule to ensure thorough documentation and care planning.
- Engage in value-based care initiatives to improve patient outcomes and support organizational goals.
- Participate in holistic health services, including coordination with in-house specialists in cardiology, podiatry, and more.
- Provide patient education, preventive care, and chronic disease management tailored to the senior population.
Salary & Benefits
- Base compensation starting at $250,000+ with opportunities for incentive bonuses up to 10% of base salary.
- Signing bonus up to $50,000.
- Pathway to partnership generally within 12-18 months, with a high rate of physicians achieving partner status after one year.

Qualifications & Requirements
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.

- Strong interpersonal, communication, and organizational skills.
