As a Primary Care Physician (PCP), you will work in a supportive, collaborative environment focused on quality patient care and professional development. You will manage a small, manageable patient panel comprised primarily of seniors living with multiple chronic conditions. Primary Care Physician (PCP) - Summary
- Full-time clinical role focused on outpatient primary care
- Serve a panel of adult and senior patients, many with chronic conditions

Duties & Responsibilities
- Provide high-quality, evidence-based primary care to a diverse senior patient population, focusing on preventive health and chronic disease management
- Conduct thorough medical assessments, develop individualized care plans, and monitor patient progress
- Manage a small patient panel (approximately 450 patients maximum) to ensure personalized, attentive care
- See up to 18 patients per day, with administrative time scheduled into your workday
- Work Monday through Friday, with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm); enjoy no nights or weekends
- Collaborate with a dedicated clinical support team, including your own medical assistant, scheduler, and referral specialist
- Leverage in-house consulting specialists in cardiology, podiatry, and other fields to optimize patient outcomes
- Incorporate holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling into patient treatment plans as appropriate
- Champion value-based care initiatives and best practices to improve overall patient health and satisfaction
- Participate in partnership opportunities within the organization, with no buy-in required and a pathway to partnership typically within 12-18 months
Salary & Benefits

Qualifications & Requirements
- Residency-trained in Internal Medicine (IM) or Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Demonstrated ability to provide compassionate, patient-centered care to adult and senior populations
- Commitment to value-based healthcare and continuous quality improvement
