Primary Care Physician (PCP) – Summary
- Deliver coordinated, world-class primary care to seniors experiencing multiple chronic conditions.
- Work in a physician-led, value-based care model with reduced patient panel sizes.
- Full-time, outpatient role with a supportive care team and robust clinic resources.
Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, primarily with 3-5 chronic diseases.
- Conduct patient visits (18-20 per day) Monday through Friday, with administrative time built into the schedule.
- Practice in a 100% outpatient setting with no night shifts or hospital call.
- Collaborate with in-house specialists in cardiology, podiatry, and more.
- Engage patients in holistic health initiatives, including acupuncture, tai chi, yoga, and nutrition services.
- Utilize dedicated support staff (MA, scheduler, referral specialist) for coordinated care delivery.
- Participate in value-based care initiatives to drive quality and cost outcomes.

Qualifications & Requirements
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM); Geriatric Medicine experience is a plus.
- Board Certified or Board Eligible in FM, IM, or Geriatrics.
- Bilingual (Spanish) preferred; Russian language skills are a plus.
- Strong commitment to patient-centered, value-based care.
