Primary Care Physician (PCP) - Summary- 100% outpatient primary care role focused on Medicare Advantage patients
- Value-based care model emphasizing quality, prevention, and chronic disease management
- Team-based approach with comprehensive support staff
- Modern, patient-centered health centers
- Full-time position based in Phoenix, AZ
- Deliver direct outpatient primary medical care to a panel of adult patients, with a focus on seniors enrolled in Medicare Advantage plans
- Manage a patient volume of approximately 14-16 patients per day, allowing for longer and more meaningful appointments
- Implement preventive care strategies and chronic disease management protocols
- Collaborate closely with a multidisciplinary team, including medical assistants, behavioral health professionals, social workers, and nurses
- Utilize electronic health records to document and coordinate patient care
- Contribute to the development and execution of individualized care plans
- Participate in quality improvement initiatives and value-based care programs
- Engage patients and their families in health education, goal setting, and shared decision-making
