Primary Care Physician (PCP) - Summary

Duties & Responsibilities
- Deliver direct outpatient primary medical care to a panel of adult patients, with a focus on seniors enrolled in Medicare Advantage plans
- Manage a patient volume of approximately 14-16 patients per day, allowing for longer and more meaningful appointments
- Implement preventive care strategies and chronic disease management protocols
- Collaborate closely with a multidisciplinary team, including medical assistants, behavioral health professionals, social workers, and nurses
- Utilize electronic health records to document and coordinate patient care
- Contribute to the development and execution of individualized care plans
- Participate in quality improvement initiatives and value-based care programs
- Engage patients and their families in health education, goal setting, and shared decision-making

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Board Certification or Eligibility in Internal Medicine or Family Medicine
- Active and unrestricted medical license
- Experience or interest in value-based care models and working with senior populations preferred

- Strong teamwork, communication, and organizational skills
- Proficient in the use of electronic health records
