Primary Care Physician (PCP) – Summary- Full-time Primary Care Physician (PCP) position focused on value-based outpatient care for senior patients
- Serve a diverse, moderate to low-income geriatric population with complex chronic conditions
- Join a physician-led, collaborative clinical culture dedicated to patient-centered care
- Work in Fort Lauderdale, FL, a thriving region with an outstanding quality of life
- Provide comprehensive primary care services to a panel of up to 450 patients
- Conduct patient assessments, develop and manage care plans, and coordinate ongoing treatment for seniors with multiple chronic diseases
- See 18-20 patients per day in a fully outpatient setting
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Utilize value-based care principles to improve patient outcomes and reduce healthcare costs
- Incorporate administrative time into your schedule to manage documentation and care coordination
- Participate in partnership opportunities, with most physicians achieving partner status within 12-18 months
- Engage with in-house consulting specialists such as cardiologists and podiatrists
- Support holistic patient wellness through services like acupuncture, tai chi, yoga, and nutrition counseling
- Competitive base salary ($250,000+), enhanced by performance-based incentive bonuses
- Signing bonus up to $50,000

- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certification or Board Eligibility in FM, IM, or Geriatric Medicine
