Primary Care Physician (PCP) – Miami, FL – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our physician-led medical group in Miami, FL. As a Primary Care Physician, you will play a crucial role in delivering coordinated, high-quality care to a panel of moderate to low-income senior patients, many of whom have complex chronic conditions. Primary Care Physician (PCP) – Summary
- Full-time position within a physician-led, patient-centered organization
- Focus on serving moderate to low-income seniors with multiple chronic diseases
- Emphasis on relationship-building and value-based care
Duties & Responsibilities
- Manage a patient panel of up to 450 seniors, typically with 3-5 chronic conditions each
- Conduct comprehensive health assessments, develop care plans, and provide ongoing management for chronic disease
- See 18-20 patients daily during regular clinic hours (Monday to Friday, 7:30 am – 5:00 pm, last patient scheduled at 4:15 pm)
- No nights or weekend shifts required; maintain work/life balance
- Participate in value-based initiatives to enhance care quality and cost outcomes
- Utilize a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Coordinate with in-house consulting specialists, such as cardiologists and podiatrists, as well as holistic services like acupuncture and nutrition
- Document all care activities accurately and efficiently within allotted administrative time
- Promote a positive and respectful patient care environment
Salary & Benefits
- Competitive base salary starting at $250,000 with additional incentive bonus opportunities (up to 10% of base salary)
- Sign-on bonus available up to $50,000
- Potential for partnership within 12-18 months (no buy-in required; 65% of physicians achieve partnership after one year)

- Continuing medical education (CME) allowance of $3,500 and one week of CME leave

Qualifications & Requirements
- Completed residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Bilingual proficiency in Spanish is strongly preferred
- Demonstrated commitment to serving senior populations with complex, chronic medical needs
- Excellent interpersonal and communication skills
- Eligibility to obtain and maintain an unrestricted medical license in Florida
