Primary Care Physician (PCP) - Summary
- Deliver comprehensive and coordinated preventive primary care to adults 65+ within a value-based care (VBC) model
- Work as part of an integrated care team including nurse practitioners, physician assistants, medical assistants, social workers, and pharmacy and referral coordination staff
- Focus on quality patient relationships and improved health outcomes, not volume-based metrics
- Participate in a robust onboarding and training program tailored to value-based care and optimized electronic medical record (EMR) usage
Duties & Responsibilities
- Provide primary care services to a predominantly Medicare Advantage patient population, focusing on adults aged 65 and older
- Conduct thorough assessments, diagnosis, and treatment planning for chronic disease management and preventive care
- Average daily patient census of 12-15 individuals with 20-40 minute visit lengths for comprehensive care delivery
- Collaborate closely with a multidisciplinary care team to ensure coordinated and effective patient care
- Participate in regular team meetings and quality improvement initiatives to enhance patient outcomes and care processes
- Utilize and optimize EMR systems (Athena, with ongoing transitions) for accurate documentation and workflow efficiency
- Support and mentor mid-level providers (NPs/PAs) and medical assistants as part of the integrated care team
- Engage in ongoing professional development and training, including a comprehensive 3-week onboarding program focused on value-based care principles and best practices

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience in geriatrics or with older adult populations preferred
- HEDIS and value-based care (VBC) experience a plus, though not required; comprehensive training provided
- Demonstrated ability to work collaboratively within a multidisciplinary care team
