We are seeking an enthusiastic Primary Care Physician (PCP) to join our team on a full-time basis. The role is 100% outpatient, focused on value-based care, and designed to support manageable patient loads and work-life balance. You will work alongside a dedicated care team including a medical assistant, scheduler, referral specialist, and receive leadership support, ensuring you can focus on providing exceptional patient care. Primary Care Physician (PCP) - Summary
- Provide comprehensive outpatient primary care services to a diverse patient population.

Duties & Responsibilities
- Deliver full-spectrum outpatient primary care to adult and senior patients.
- Manage a patient panel with an average of 12-14 patients per day (maximum panel size: 650).
- Work collaboratively with a care team that includes a medical assistant, scheduler, and referral specialist.
- Adhere to quality benchmarks including HEDIS compliance and patient satisfaction goals.
- Participate actively in company values and contribute to reducing hospital sick days among patients.
- Leverage transportation and wellness program offerings to enhance patient engagement and outcomes.
- Maintain accurate and timely documentation of patient encounters and medical records.
- Communicate effectively with patients, families, and other healthcare professionals to coordinate care plans.
Salary & Benefits
- Competitive base salary ranging from $265,000 to $290,000 annually.
- Annual performance bonus up to 20% of base salary based on patient satisfaction, company values, HEDIS compliance, and reduced hospitalizations.
- Sign-on bonus of $75,000.

- 401(k) plan with options for pre-tax or Roth contributions and up to 4.5% company matching.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board certification or eligibility in Family Medicine or Internal Medicine.
- Active and unrestricted medical license.
- Commitment to value-based, patient-centered care models.
- Excellent communication, interpersonal, and organizational skills.
- Experience working in outpatient primary care settings preferred.
- Demonstrated ability to work effectively in a team-oriented environment.
