Primary Care Physician (PCP) - Summary
- Deliver outpatient, value-based primary care to a senior patient population.
- Work within a physician-led, mission-driven organization focused on patient well-being and clinical excellence.
- Be supported by a dedicated care team and administrative staff to optimize patient outcomes and provider satisfaction.
Duties & Responsibilities
- Provide comprehensive primary care services to a panel of approximately 450 patients, primarily seniors with multiple chronic conditions.
- Conduct patient visits (18-22 per day) and manage treatment plans tailored to individual health needs.
- Collaborate with in-house consulting specialists in areas such as cardiology and podiatry.
- Work closely with holistic health providers offering services like acupuncture, Tai Chi, yoga, and nutrition counseling.
- Utilize value-based care models to improve health outcomes and patient satisfaction.
- Ensure all care is delivered in accordance with established clinical protocols and best practices.
- Participate in ongoing professional development and contribute to continuous quality improvement initiatives.
- Maintain accurate and timely documentation in the electronic medical record system.
- Benefit from administrative time built into your schedule for thorough patient follow-up and care coordination.
- Coordinate patient transportation services as needed to reduce barriers to care.

Qualifications & Requirements
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
