Primary Care Physician (PCP) - Summary- Provide comprehensive, patient-focused outpatient care to adult and senior populations
- Collaborate with a multidisciplinary team to ensure quality and continuity of care
- Focus on preventive medicine, chronic condition management, and population health

- Deliver primary medical care to adult and senior patients in an outpatient setting
- Diagnose and manage acute and chronic medical conditions

- Participate in daily huddles, team meetings, and ongoing process improvement initiatives
