Primary Care Physician (PCP) - Summary
- Deliver comprehensive, patient-centered primary care to a panel of seniors with complex health needs
- Work within a physician-led, value-based care model focused on improving health outcomes

Duties & Responsibilities
- Provide outpatient primary care services to a panel of approximately 450 patients
- Manage and coordinate care for seniors with multiple chronic conditions, focusing on prevention and quality of life
- Conduct thorough patient assessments, develop care plans, and monitor treatment progress
- Collaborate with in-house consulting specialists (Cardiology, Podiatry, etc.) and holistic health providers (Acupuncture, Tai Chi, Yoga, Nutrition)
- Utilize an integrated care team, including a dedicated Medical Assistant, Scheduler, and Referral Specialist, to optimize patient care delivery
- Promote value-based care principles, ensuring high-quality and cost-effective services
- Document patient encounters, care plans, and follow-ups accurately and efficiently
- Participate in regular administrative time and team meetings to support clinical operations
- Maintain compliance with all regulatory, legal, and ethical standards
- Contribute to the organization’s mission and support continuous quality improvement initiatives
Salary & Benefits

Qualifications & Requirements
- Residency-trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in one of the above specialties
- Strong commitment to value-based and patient-centered care
- Excellent communication, organizational, and teamwork skills
- Demonstrated ability to manage complex patient populations
- Dedication to continuous learning and professional development
- Eligibility to obtain and maintain an unrestricted medical license
