Primary Care Physician (PCP) - Summary
- Full-time role providing primary care services to a senior patient population in Columbus, OH.

Duties & Responsibilities
- Deliver comprehensive primary care services to a panel of up to 450 senior patients, managing an average of 18-22 patients daily.
- Focus on value-based care and chronic disease management, primarily serving seniors with three to five chronic conditions.
- Collaborate with a dedicated care team including a medical assistant, scheduler, and referral specialist to ensure seamless care delivery.
- Maintain clinic hours Monday through Friday, with administrative time incorporated into your schedule (no nights or weekends required).
- Engage in partnership opportunities, typically attainable within 12-18 months, with a significant percentage of physicians achieving partnership within their first year.
- Utilize the clinic’s in-house consulting specialists in cardiology, podiatry, and other areas to coordinate holistic patient care.
- Support patients’ overall well-being by integrating holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Ensure continuity of care with door-to-door transportation provided for patients as needed.
Salary & Benefits
- Competitive base salary starting at $250,000, with additional incentive and signing bonuses available.
- Potential for up to 10% of base salary as an annual performance bonus.

Qualifications & Requirements
- MD or DO degree from an accredited institution.
- Board certified or board eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.

- Strong collaborative skills and an ability to work effectively within an interdisciplinary care team.
