Primary Care Physician (PCP) - Summary- Full-time position supporting Medicare patient populations in a community-focused, value-based primary care setting
- Collaborative team environment with a focus on holistic patient outcomes
- Equipped with on-site laboratory, access to specialist consults, and robust EMR platforms
- Provide comprehensive primary care services to adult and senior patients, focusing on preventive care and chronic disease management
- Engage with patients through regular visits (20-minute standard, 40-minute for new/complex cases)
- Work as part of a multidisciplinary team including scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for accurate documentation and care coordination
- Collaborate with specialists via RubiconMD and ensure seamless care transitions
- Participate in community wellness initiatives and patient engagement activities
- Uphold the principles of value-based healthcare, prioritizing quality metrics and positive patient outcomes
- Maintain compliance with all regulatory and ethical standards

- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution
- Board certification (or eligibility) in Internal Medicine or Family Medicine
- Active, unrestricted license to practice medicine in the state
- Fellowship training in Geriatrics preferred but not required
- Applicants must not be current residents
- Experienced in primary care, internal medicine, or family medicine
- Passionate about value-based healthcare and preventive medicine
- Comfortable working in a multidisciplinary, team-based environment
- Strong communication, documentation, and organizational skills
- Committed to delivering equitable, compassionate, and high-quality patient care
- Adaptable and enthusiastic about participating in community health initiatives
- State-of-the-art clinic facilities with on-site labs and community activity rooms
- Patient transportation available within a five-mile radius of the clinic
- Average daily patient volume of 13-16, prioritizing quality of care
- Clinics staffed with a mix of 3-5 providers, including Nurse Practitioners and Primary Care Physicians (PCP)
