Primary Care Physician (PCP) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our physician-led, mission-driven primary care organization in Palm Beach County, FL. As a Primary Care Physician, you will play a vital role in delivering high-quality, value-based outpatient care to a panel of senior patients. Key Responsibilities
- Deliver comprehensive outpatient primary care services to a panel of senior patients, typically managing 3-5 chronic diseases per patient.
- Conduct patient visits in a clinic setting, with a manageable daily volume of approximately 18 patients.
- Coordinate care with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient experiences.
- Engage in value-based care practices, focusing on quality and outcomes rather than volume.
- Collaborate with in-house consulting specialists in fields such as cardiology and podiatry to deliver holistic care.
- Participate in holistic health services offered by the clinic, including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Utilize provided administrative time within your schedule to complete necessary documentation and follow-up tasks.
- Maintain a work schedule of Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- Work exclusively in an outpatient setting with no night or weekend shifts required.
- Contribute to the organization's mission of improving the health and well-being of seniors in the community.
Qualifications
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing patient-centered and value-based care.
- Strong interpersonal and communication skills to effectively engage with patients, families, and care team members.
