Primary Care Physician (PCP) - Summary- Provide comprehensive outpatient primary care to a senior patient population.
- Collaborate with a multidisciplinary support team dedicated to value-based care.
- Emphasize preventative medicine and chronic disease management.
- Opportunity to grow into a partnership role within the organization.
- Manage a patient panel of up to 450 patients, focusing on seniors with complex chronic conditions.
- Conduct thorough assessments, develop treatment plans, and monitor patient progress.
- Work a Monday through Friday schedule with clinic hours from 7:30am to 5:00pm; last patient scheduled at 4:15pm.
- Utilize administrative time included in the schedule for care coordination and documentation.
- Collaborate closely with in-house consulting specialists, including cardiology and podiatry.
- Leverage holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling for patient benefit.
- Participate in value-based care initiatives and quality improvement programs.
- Engage in ongoing professional development and support the mission of delivering better health to seniors.

- Residency-trained in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
