Primary Care Physician (PCP) - Full-Time
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our innovative healthcare team. As a Primary Care Physician (PCP), you will see a manageable daily patient volume, allowing for thorough and attentive care. The patient panel primarily consists of older adults with complex, chronic conditions such as heart disease, diabetes, and hypertension. You will have the opportunity to positively impact patient outcomes, reduce hospitalizations, and participate in a value-based care model that rewards quality of care, patient satisfaction, and adherence to clinical best practices.
The ideal candidate will be board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM), with a strong desire to work in a collaborative, mission-driven setting. A fellowship in geriatrics is a plus but not required. The position requires an active, unrestricted medical license and current DEA registration. Proficiency in basic office-based procedures, such as suturing, joint injections, skin biopsies, and wound care, is essential.

- Maintain thorough and accurate patient records in compliance with regulatory standards
- Participate in shared call schedule with other providers as assigned
