Primary Care Physician (PCP) team members benefit from a collaborative, patient-first approach. Our outpatient-only model means no nights or weekends, and administrative time is built into your schedule, allowing you to deliver the highest standard of care to each patient. The patient population primarily consists of seniors managing multiple chronic conditions, providing you with the opportunity to make a meaningful impact through value-based medicine. Onsite access to consulting specialists in cardiology, podiatry, and more, along with holistic health services like acupuncture, tai chi, yoga, and nutrition counseling, supports a comprehensive care approach. Door-to-door transportation is available for patients, further removing barriers to care.
We are seeking Primary Care Physician (PCP) professionals who are board certified or board eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine. Residency training in IM or FM is required. Primary Care Physician (PCP) – Summary- Full-time position focused on outpatient, primary care for seniors
- Physician-led, mission-driven organization
- Collaborative team environment with robust administrative and clinical support
- Located in vibrant New Orleans, LA, offering a unique and rewarding practice experience
- Provide comprehensive primary care to a panel of approximately 450 senior patients (maximum)
- Manage a daily schedule of 18 patients, Monday through Friday
- Utilize value-based care models to address chronic conditions and promote better health outcomes
- Collaborate with in-house consulting specialists and a dedicated care team
- Participate in holistic care initiatives, including referrals for acupuncture, tai chi, yoga, and nutrition counseling
- Document patient care accurately and efficiently with administrative support
- Ensure timely referrals and continuity of care for complex cases
- Engage in ongoing professional development and partnership opportunities within the organization
