Primary Care Physician (PCP) – Summary
- Deliver outpatient, value-based primary care to a senior patient population.
- Work collaboratively with a dedicated care team, including your own medical assistant, scheduler, and referral specialist.
- Maintain a patient panel of up to 450 individuals, ensuring personalized, high-quality care.
- No nights or weekends required – maintain a balanced work schedule.

Duties & Responsibilities
- Conduct comprehensive primary care visits for seniors with complex medical needs, typically managing 3-5 chronic diseases per patient.
- See approximately 18 patients per day, focusing on quality and continuity of care.
- Participate in value-based care initiatives to improve patient outcomes and satisfaction.
- Utilize scheduled administrative time for effective care coordination and documentation.
- Collaborate with in-house consulting specialists, including cardiology and podiatry, as well as holistic health service providers offering acupuncture, Tai Chi, yoga, and nutrition counseling.
- Engage in partnership opportunities after 12-18 months, with a significant percentage of physicians achieving partnership status within the first year.
- Ensure compliance with all clinical protocols and organizational standards.
- Maintain accurate, timely medical records in alignment with regulatory requirements.

Qualifications & Requirements
- Completion of residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Commitment to delivering value-based, patient-centered care.
- Strong organizational and communication skills.
- Ability to work both independently and collaboratively within a care team.
- Dedication to continuous learning and improvement.
