Primary Care Physician (PCP) – Summary- Full-time role focused on comprehensive, value-based primary care for seniors

- Provide outpatient primary care for a panel of up to 450 patients (senior population)
- Conduct patient assessments, develop care plans, and manage chronic conditions (3-5 chronic diseases typical per patient)
- Collaborate with a dedicated care team including MA, scheduler, and referral specialist
- Utilize value-based care principles to optimize patient outcomes
- Participate in administrative tasks with protected time in schedule
- Work Monday to Friday, clinic hours 7:30 am – 5:00 pm (last patient scheduled at 4:15 pm)
- Maintain work/life balance with no night or weekend shifts
- Engage with in-house consulting specialists and holistic health services for comprehensive patient care
