Primary Care Physician (PCP) - Summary
- Full-time, outpatient primary care role focused on delivering value-based care to seniors

Duties & Responsibilities
- Provide comprehensive medical care to a panel of seniors, typically managing 3-5 chronic illnesses per patient
- Conduct thorough patient assessments, diagnose illnesses, and develop individualized care plans
- See an average of 18 patients per day, allowing for quality time with each individual
- Collaborate with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Work closely with in-house consulting specialists such as cardiology and podiatry for coordinated patient care
- Participate in holistic health offerings, including acupuncture, tai chi, yoga, and nutrition counseling
- Ensure compliance with value-based care standards and organizational protocols
- Utilize administrative time included in your schedule for patient documentation and care coordination
- Maintain a strong focus on preventive care and chronic disease management
- Engage in regular professional development and quality improvement initiatives
Salary & Benefits
- Competitive base salary with annual incentive bonuses

Qualifications & Requirements
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine

- Strong communication, interpersonal, and organizational skills
- Ability to work collaboratively within a multidisciplinary team

- Clinic hours are Monday through Friday, with last patient appointments scheduled at 4:15 PM
- No night or weekend shifts required
