Primary Care Physician (PCP) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our mission-driven healthcare team in New Orleans, LA. As a Primary Care Physician, you will play a vital role in providing accessible and high-quality care in a supportive, collaborative environment.
Responsibilities
- Provide comprehensive primary care services for a patient panel of up to 450 individuals, focusing on seniors with 3-5 chronic diseases.
- Conduct patient assessments, develop treatment plans, and manage ongoing care for a diverse senior population.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated patient care.
- Utilize value-based care models to improve patient outcomes while managing healthcare costs effectively.
- See an average of 18 patients per day in an outpatient clinic setting.
- Maintain thorough and timely documentation within the electronic health record system.
- Participate in partnership opportunities within the organization, with many physicians achieving partnership within 12-18 months.
- Engage in ongoing professional development and participate in organizational initiatives to enhance the quality of care.
- Work closely with in-house consulting specialists in disciplines such as cardiology and podiatry, as well as holistic health services including acupuncture, tai chi, yoga, and nutrition.
Qualifications
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Strong commitment to delivering patient-centered, value-based care.
- Excellent communication and interpersonal skills, with the ability to work collaboratively within a multidisciplinary team.
Schedule and Work Environment
- Full-time, outpatient position.
- Clinic operates Monday through Friday, 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
- No night or weekend shifts required, supporting a healthy work-life balance.
- Administrative time is incorporated into the daily schedule.
