Primary Care Physician (PCP) - Summary- Full-time Primary Care Physician (PCP) position focused on serving a senior patient population
- Located in New Orleans, LA, within a mission-driven, physician-led healthcare organization
- Emphasis on value-based, comprehensive outpatient care

- Deliver primary care to a patient panel of up to 450 seniors with complex chronic conditions
- See approximately 18 patients per day in an outpatient clinic setting
- Work closely with a dedicated care team including medical assistants, schedulers, and referral specialists
- Participate in a Monday–Friday clinic schedule with no nights or weekends required
- Maintain detailed patient records and contribute to quality improvement initiatives
- Utilize in-house consulting specialists (e.g., Cardiology, Podiatry) as needed for comprehensive patient care
- Incorporate holistic health services and wellness programming into patient care plans
- Ensure exceptional patient service and adherence to value-based care principles
- Engage in administrative duties with built-in schedule time for documentation and care coordination
