Primary Care Physician (PCP) - Summary- Full-time Primary Care Physician (PCP) role in Portsmouth, VA
- Physician-led, mission-driven healthcare organization

- Provide direct outpatient primary care services to a panel of senior patients, managing a maximum of 450 patients
- Deliver patient-centered care, focusing on prevention, chronic disease management, and wellness initiatives
- Work collaboratively with a dedicated medical assistant, scheduler, and referral specialist for coordinated care delivery
- Utilize a value-based care model to optimize health outcomes for patients with 3-5 chronic diseases
- Maintain clinic hours Monday through Friday, with no night or weekend shifts required
- Participate in regular administrative time included in the daily schedule to ensure thorough patient care and documentation
- Access in-house consulting specialists in cardiology, podiatry, and more, as well as holistic services such as acupuncture, tai chi, yoga, and nutrition counseling
- Support organizational mission by delivering affordable, high-quality VIP care to the neediest populations in Portsmouth, VA
