As a Primary Care Physician (PCP) in our Atlanta, GA clinic, you will work alongside experienced medical assistants, schedulers, referral specialists, and a multidisciplinary team, all committed to supporting your practice. Primary Care Physician (PCP) - Summary
- Full-time, outpatient primary care position serving an adult and senior population
- Based in a physician-led, mission-driven organization dedicated to value-based care
- Focus on smaller patient panels for enhanced care delivery
- Opportunities for partnership typically available within 12-18 months
Duties & Responsibilities
- Provide comprehensive primary care for a panel of adult and senior patients, many with 3-5 chronic medical conditions
- See an average of 18 patients per day in a supportive, team-based environment
- Develop, implement, and monitor personalized care plans with an emphasis on preventive health and chronic disease management
- Collaborate with in-house specialists in cardiology, podiatry, and other disciplines to optimize patient outcomes
- Participate in value-based care initiatives to improve quality and efficiency
- Utilize available holistic health services such as acupuncture, tai chi, yoga, and nutrition to support patient wellness
- Work closely with dedicated medical assistants, schedulers, and referral specialists to coordinate care
- Ensure thorough documentation and compliance with clinical best practices and organizational standards
- Engage in administrative time included in your schedule for care coordination and patient follow-up
- Foster a positive, inclusive, and patient-centered clinic environment

Qualifications & Requirements
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Commitment to practicing patient-centered, value-based care
- Strong communication and interpersonal skills
- Dedication to working with seniors and those with complex medical needs
